Telehealth Informed Consent

1. What telehealth is

Telehealth is the delivery of health care using secure electronic communication rather than an in-person visit. Through Therisse, a brand operated by Ironoak Holdings LLC, you are connected with independently licensed, board-certified physicians who provide care through MDI Medical Group PC's telehealth platform. Your physician may review your intake responses, photos, and lab results, communicate with you through secure messaging, and, where appropriate, prescribe treatment, all remotely.

2. Benefits of telehealth

Telehealth can give you access to licensed physicians without travel, offer greater scheduling flexibility, and let you manage ongoing hormone or weight-management care from home. It is designed to make care more convenient while keeping a licensed physician responsible for clinical decisions.

3. Risks and limitations

Telehealth also has limitations you should understand before consenting:

  • Your physician evaluates you without a physical examination and relies on the accuracy and completeness of the information you provide. Incomplete or inaccurate information can affect the quality of care.
  • In some cases the information available electronically may not be enough to make a diagnosis or prescribe treatment, and your physician may determine that in-person evaluation is necessary.
  • Technical problems, such as interruptions or equipment failure, can affect or delay communication.
  • Despite reasonable security safeguards, no electronic transmission of information is completely secure, and there is a small risk that protected health information could be accessed without authorization.

4. Telehealth is not for emergencies

Telehealth is not a substitute for emergency care. If you are experiencing a medical emergency, call 911 immediately or go to the nearest Emergency Department. Telehealth is also not a substitute for a relationship with a primary care provider, and your physician may recommend that you seek in-person care where appropriate.

5. Physician discretion

There is no guarantee that you will be treated by a participating physician or that a prescription will be issued. Whether treatment is appropriate, and whether a prescription is appropriate, is determined in the professional judgment of the treating physician. A physician may decline to provide care or a specific treatment if, in their professional judgment, it is not medically or ethically appropriate.

6. Your rights

You have the right to:

  • Ask questions about telehealth and about any recommended treatment before agreeing to it
  • Withhold or withdraw your consent to telehealth at any time, without affecting your right to future care or treatment
  • Request an in-person referral instead of, or in addition to, telehealth
  • Expect that the laws protecting the privacy of your medical information apply to telehealth

Withdrawing consent to telehealth going forward does not undo care already provided. To withdraw consent, email support@therisse.com.

7. Communications consent

By proceeding, you consent to your treating physician and their support staff communicating with you through secure in-platform messaging and the other channels described in our Terms of Service. Secure in-platform messaging remains a required channel for clinical communications while you have an active treatment plan.

8. Acknowledgment

By completing the medical intake form and using the service, you acknowledge that you have read and understood this consent, that you have had the opportunity to ask questions, and that you consent to receive care through telehealth. Questions can be sent to support@therisse.com.
